Ball-Shaped Thrombi in the Left Ventricular Apex

چکیده
A 67-year-old man was admitted to the cardiology department because of exertional dyspnea and orthopnea aggravated for 10 days. Transthoracic echocardiography showed dilated left ventricle (LV) with global hypokinesia with severely decreased systolic function (ejection fraction was estimated to be about 15%). Two oscillating masses with smooth, spherical contour were found in the LV apex (Fig. 1A). The size of masses were estimated 22 by 29 mm and 14 by 18 mm respectively. The patient received unfractionated heparin intravenously for 5 days, followed by oral warfarin therapy. On the 8th hospital day, follow-up echocardiography revealed partial resolution of thrombi with resultant highly movable friable remnants (Fig. 1B). One the 18th hospital day, follow-up echocardiography revealed nearly complete resolution of thrombi (Fig. 1C). The patient was discharged home uneventfully. Ball shaped-masses in LV may be thrombi, vegetations or tumors. Although echocardiography is the procedure of choice for the diagnosis of cardiac mass, differentiation between myxoma and thrombus may be very difficult if the mass size is small, contours are smooth, or attachment site is atypical or ill-defined. Short-term anticoagulation therapy, which can differentiate thrombi from tumors, makes unnecessary surgical procedure avoidable.

BACKGROUND Ventricular thrombus formation is a frequent and potentially dangerous complication in patients with ischemic heart disease. Although transthoracic echocardiography (TTE) is generally used as diagnostic technique, we explored the role of contrast-enhanced (CE)-MRI to detect ventricular thrombi. METHODS AND RESULTS In 57 patients with acute myocardial infarction, chronic myocardial ...

A 64-year-old man with no significant medical history presented to the emergency department with severe acute heart failure. Initial management was based on oxygen therapy, diuretics, and nitroglycerin. Twenty-four hours after admission, the patient developed aphasia and left arm weakness. Brain MRI showed multiple strokes (Figure A) compatible with cardiac emboli. Echocardiography showed dilat...

Thrombi represent the most frequently found intracardiac masses. Left ventricular thrombus (LVT) is an important complication in patients with ischemic heart diseases and in those with dilated cardiomyopathy and systolic heart failure. The diagnosis of left ventricular thrombus remains important since anticoagulation will reduce the risk of systemic embolization and stroke. Despite advances in ...
متن کاملLeft Ventricle: Echocardiographic Detection and Differentiation from Early Thrombosis
In two-dimensional echocardiographic studies of left ventricular thrombus in patients, an unusual pattern of dynamic left ventricular intracavitary echoes was identified in some hearts with severe apical dysfunction. These intracavitary echoes were noted in the apical region and were distinct from left ventricular thrombi. Certain features of the intracavitary echoes suggested that they were ge...
